King, a 15-year-old Belgian gelding and “Resident Ambassador” for Gentle Giants Draft Horse Rescue, is back to his lively self after a complex dental and sinus procedure at the Marion duPont Scott Equine Medical Center in Leesburg, Virginia.
The horse was referred to the center in August 2025 after developing a foul-smelling discharge from his left nostril. Radiographs revealed an infection originating from a fractured molar that had spread into his sinus, prompting a CT scan for more detailed evaluation.
Dr. Elsa Ludwig, clinical assistant professor of equine surgery, performed a standing extraction of the fractured tooth and a sinusotomy to remove infected material and improve drainage.
Assisted by intern veterinarian Antonio Beck Jr., Ludwig removed tooth fragments, flushed the cavity, and placed a temporary dental plug. The sinuses were cleaned, lavaged with sterile saline, and treated with medical-grade Manuka honey for its antimicrobial properties.
During hospitalization, King received antibiotics and regular sinus lavages. After six days, the dental plug was replaced and he returned home for continued care. A follow-up visit revealed significant healing, with healthy granulation tissue and only minor residual debris in the sinuses.
“Treating dental and sinus diseases in horses is extremely rewarding because it immediately improves their comfort,” Ludwig said. She emphasized the importance of regular dental care to prevent serious health issues and highlighted the center’s ability to diagnose and treat these conditions using radiographs, CT scans, and endoscopy.
King continues to serve as a gentle ambassador for the rescue, educating the public and participating in events, now fully recovered and enjoying his retirement alongside his longtime companion, Ada.
