Tooth Extraction at Granite City Home Leads to ER Visit, Two Face Felony Charges

GRANITE CITY, Ill. — Authorities say a quiet Granite City residence secretly operated as an underground dental clinic, resulting in at least one patient requiring emergency care and two residents facing felony charges.

Court documents indicate that Idania J. Morena-Paal, 41, and Rudolfo J. Figuera, 59, conducted dental procedures in Morena-Paal’s home, including a tooth extraction on Sept. 6. A patient who paid $850 later developed a serious infection, prompting visits to Touchette and Alton Memorial hospitals.

Both Morena-Paal and Figuera were charged Feb. 24 with practicing medicine without a license, and Morena-Paal faces an additional count. Investigators seized dental tools, medications, cash, phones, ledgers, and bank statements from the home. Both defendants have been released from custody.

Illinois law strictly prohibits practicing medicine without a license. A first-time violation is typically treated as a Class 4 felony, with stricter penalties for repeat offenses. The law also allows civil fines and court orders to halt unlicensed medical work that endangers patients.

Investigators say the case emerged after a patient reported dental care from Morena-Paal between mid-August and Sept. 6. The infection that followed necessitated additional emergency treatment. Court papers note that Morena-Paal allegedly posted graphic photos of the procedure online.

Experts note that high costs and limited access to dental care often push individuals toward unregulated treatments, particularly in low-income and immigrant communities. Coverage gaps and provider shortages can drive patients to risky alternatives that may result in severe health complications.

The case will proceed through Madison County courts, with prosecutors reviewing filings and scheduling hearings. Individuals who received treatment from Morena-Paal or Figuera, or who have relevant information, are urged to contact local law enforcement.
