Doug’s Pediatric Dental Practice (the “Practice”) recently experienced a data security incident that may have involved personal and/or protected health information.
The Practice takes the privacy and security of all information it maintains very seriously. We have notified potentially impacted individuals and provided resources to assist them.
In September 2024, Dr. Doug noticed unusual activity in one of his email accounts. The Practice immediately launched an investigation but was unable to determine if any emails had been accessed without authorization.
The investigation confirmed that only the affected email account was compromised; no other systems or networks in Dr. Doug’s environment were affected.
As a precautionary measure, Dr. Doug reviewed all emails in the affected account to identify any personal or protected health information and collected the contact information necessary to notify relevant individuals.
This review was completed in June 2025, and letters were sent to all potentially impacted individuals who provided mailing addresses.
Based on the findings of this review, the incident may have involved information such as names, dates of birth, details of dental diagnoses or treatments, and/or Medicaid or dental insurance information.
In rare cases, Social Security numbers or driver’s license information may also be affected. It’s important to note that not all information for all individuals was affected.
Upon discovery of this incident, Dr. Doug immediately implemented these measures and further strengthened the security of his email environment.
We have also established a toll-free call center to answer questions and address concerns related to this incident.
